Medication Information for Washington DC: If your son or daughter needs medication administered to them during and throughout the Washington DC Trip in October, the "DC Medication Form for Parents" needs to be filled out by a PHYSICIAN and a PARENT (the form is attached to this website).  Even though this is the same form that is turned into the office for school related medication that is administered, a SEPARATE FORM must be filled out and on file for the Washington DC trip.  These need to be turned into the office when filled out or given to Mrs. Krueger.  If you have any questions regarding the Washington DC trip and the medication process, please contact Mrs. Krueger at (614) 801-3900 (ask to speak to Mrs. Krueger) or email me at Jeannie.Krueger@swcs.us.  The best time to reach me by phone is during my planning (2012-2013 school year is from 3:00-4:00 and the 2013-2014 school year is from 1:00-2:00).
